| Video Discription | A 41 year old man developed movement disorders after rapid correction of gastroenteritis-induced hyponatremia (serum sodium = 117 mEq/L (to convert to millimoles per liter, multiply by 1), corrected with IV normal saline to 128 mEq/L within 24 hours) with subsequent osmotic demyelination syndrome (ODS). Brain MR imaging confirmed central pontine and extra-pontine myelinolysis. He presented initially with parkinsonism, was treated with levodopa, and then developed generalized chorea affecting all 4 limbs with gait abnormality, as illustrated in this video. Striatal involvement in ODS is common and can be associated with a variety of hypokinetic and/or hyperkinetic movement disorders; delayed occurrence is thought to be attributable to denervation supersensitivity and/or aberrant neuronal reorganization.
