| Video Discription | Try to diagnose the case on the basis of echocardiography clips shown in the start of the video. Spot diagnosis is given in the second half of video with recap of echo clip so you can relate it well. Case 1: FINDINGS & DIAGNOSIS: Apical two chamber, three chamber, four and five chamber views are shown which clearly indicates hypertrophy at the apical region, followed by color doppler at apex which is showing narrowed Left ventricle cavity with turbulence in it. Continous wave doppler at apical LV cavity confirms high gardients which are DYNAMIC, suggesting apical hypertrophic cardiomyopathy (HCM) also known as Yamaguchi syndrome. Yamaguchi syndrome or apical hypertrophic cardiomyopathy (HCM) is a unique variant of HCM. It is characterized by localized hypertrophy involving the left ventricular apex rather than the left ventricular septum. This syndrome has been traditionally seen in the Asian population, particularly those of Japanese descent.
