| Video Discription | The frontal recess refers to a small space within the upper aspect of the anterior ethmoid sinus, through which mucus drains from the frontal sinus (forehead sinus). If the frontal recess becomes blocked by tissue swelling, scarring, or an obstructive lesion like a tumor, mucus will back up and accumulate in the frontal sinus, and this can cause debilitating symptoms and complications for patients. Additionally, lesions in the frontal recess and grow to involve adjacent orbital structures (eye socket) or the skull base (bony junction between the sinuses and the brain), and can therefore cause issues like blurred vision, headache, spinal fluid leak, and meningitis. In this case presentation, the patient presented with recurrent left-sided forehead pain and pressure, but she was diagnosed before she could develop the more severe complications previously mentioned. Final pathology of the surgical specimen after resection was found to be most suggestive of an arteriovenous malformation, which is very rare to find in the paranasal sinuses. An arteriovenous malformation is a benign lesion, defined as an abnormal collection of arteries sending blood directly to veins without going through tissue capillaries. While they are not technically tumors, they do grow over time as they develop more blood vessels and more blood flowing through them. These lesions are surrounded by a thin capsule separating the lesion from normal tissue, but the capsule is not robust and can be adherent to the surrounding normal tissue. Surgical resection requires meticulous dissection to avoid injury to the normal surrounding tissue. In this case, the lesion had expanded and caused erosion of the adjacent bony walls of the orbit and skull base, while causing thickening of other areas along the skull base. The lesion was carefully dissected away from these normal tissues successfully, and the sinuses healed very nicely postoperatively without any recurrence of the lesion. [kww9Is5Rspw] |
