| Video Discription | Kaiser Permanente Surgical Coaching Maxillary Antrostomy: Step 1) Local anesthetic injection - Use 0-degree endoscope and 25 gauge needle to inject 1% lidocaine with 1:100,000 epinephrine at the superior attachment of the middle turbinate to the lateral nasal wall (1mL) and anterior head of the middle turbinate (1mL) Step 2) Identify and medialize the uncinate process - Use angled ball tip probe Step 3) Remove the uncinate process and identify the natural ostium - Use backbiter. Start at 12 o’clock position in middle meatus, then pronate wrist to slide behind the free edge of uncinate. Use microdebrider to excise stray mucosal edges. Use angled endoscope (30 or 45 degree) to identify natural ostium. - Make sure you are able to incorporate the natural os of the maxillary sinus into your antrostomy. This will reduce the risk of recirculation and reoperation. The natural os is usually visible and is superior and anterior, tucked under the uncinate process. Step 4) Remove the superior uncinate process - Reflect uncinate anteriorly and then excise with upbiting through cut forceps. Step 5) Enlarge the antrostomy by removing the posterior fontanelle - Use straight through cutters to enlarge maxillary antrostomy posteriorly and incorporate posterior fontanelle if present - Although we usually enlarge the maxillary antrum, the size of the antrostomy is less important than incorporating the natural os. Step 6) Remove the inferior uncinate process - Use downbiters or straight microdebrider to enlarge the maxillary antrostomy inferiorly to the superior border of the inferior turbinate Step 7) Irrigate maxillary sinus or debride polyps as necessary - Use angled suctions and microdebriders as necessary.
