| Video Discription | Understanding the mechanisms behind the way the brain protects and repairs itself, as well as what happens when this goes wrong, is key to finding life-changing therapies for people with conditions like multiple sclerosis (MS). But the complexity of the human brain, and the challenges in accurately measuring the impact of potential treatments requires a highly multi-disciplinary and translational approach. At UCSF, the Division of Neuroimmunology and Glial Biology has long been a home for this kind of approach, connecting basic science to clinical care for people living with MS and other diseases, spanning everything from molecular mechanisms in remyelination to phenotyping of disease progression. Today though, the hopes for the coming age of brain repair are closer than ever through a range of promising research projects. Damage to myelin, the protective insulation around nerve fibers, caused by MS leads to delays in nerve signals in the brain and central nervous system, and a cascade of MS symptoms. A decade ago, UCSF researchers discovered that an over-the-counter antihistamine could play a role in remyelination. The challenge was proving that it did using biological evidence in addition to patient reported outcomes – and in 2023 results of the REbuild trial were published, using MRI data to measure myelin repair using the myelin water fraction - water trapped between the thin layers of myelin that wrap nerve fibers. Though the drug used in the trial may not be effective enough to be a viable treatment for patients with MS, the knowledge gained in the trial on how to measure myelin and the mechanisms behind it are key to ongoing research within the Division. The team are working on a multitude of other translational research projects, including developing novel strategies for neuroprotection and repair in the fetal and neonatal brain, improving phenotyping of MS progression by studying cerebrospinal fluid (CSF) and blood-derived biomarkers, as well as looking at regulatory factors relevant in other human myelin disorders like Cerebral palsy. At the same time, bringing these new development and discoveries to patients is key, with the team and their collaborators working closely with patients to help better understand the disease and bring new therapeutics and tools to these patients. With the coming age of brain repair, we can go beyond overcoming disability, to addressing the many smaller challenges that people living with MS and other neurological conditions face, and change lives for many years to come.
