| Video Discription | AST and ALT are two most frequently measured transaminases by the clinical biochemistry laboratory. Serum AST, ALT levels, and AST/ALT ratio are biomarkers for liver health. The enzymes are used in the differential diagnosis of various liver diseases and the ratio of the two enzymes provides additional clinical insight. These tests are part of LFT panel. The Estimation of transaminases is popularly done by U.V. Kinetic method at 340nm. Enzyme activity is expressed in U/L. The normal range in serum is 10-40 U/L. The enzyme concentration rises with hepatocyte damage in various conditions. Substrate depletion rate is calculated and the graph obtained is an inverse graph as the substrate is getting used with time. Here the substrate is NADH and it is forming NAD. NADH is donating its proton to Pyruvate to be converted to Lactate by Enzyme PDH. So the absorbance decrease is related to increased enzyme activity Estimation of serum ALT activity is routinely used to screen blood donors at risk of transmitting hepatitis, particularly the non-A, non-B type, since no specific serologic test is available. this acts as a surrogate test for non-A non-B hepatitis and reduces the incidence of post-transfusion hepatitis in recipients.
